In accordance with federal regulations and the University policy, all covered persons are required to enroll in the Occupational Health and Safety Program (“OHSP”) or provide certification that they are participants in a similar program at their home institutions.   Covered persons include faculty, staff and students, and bona-fide volunteers who will be working with or have a reasonable expectation of exposure to animal tissues, fluids, secretions and/or excretions (collectively referred to as "exposure to animals") while involved in research and/or education projects at on or off-campus locations.

Students MUST bring the above forms and/or an Immunization Record if applicable. If parents or students do not have copies of immunization records, it is recommended they contact their high school. If this is also not possible, medical records in Taylor Health and Wellness will help individuals obtain these records if they sign a release.

BioHazardous Addendum:

The use of animals in studies conducted with hazardous agents requires special considerations. These studies must be reviewed and approved by the IACUC and Biosafety Committees. Animal experiments with hazardous agents will have formal written safety programs, and safeguards for control of the agent. The principal investigator must ensure that their staff is competent and have adequate facilities to carry out the research safely. The use of certain hazardous agents necessitates compliance with federal, state, and local regulations and with guidelines issued by granting institutions.

The following application form is to be used to indicate the hazardous materials (agents) being used in the respective studies.
